Information you’ll need to provide

  • A completed application form (be sure to sign, notarize and date).
  • A completed owner/landlord application package (to be completed by your landlord).
  • Proof of income for everyone 18 and older in the household. 
  • Your current lease to show you've been living in the residence for at least a year before the submittal of the application. 
  • Identification  for all persons currently residing in the household, like a Driver’s License, Passport, Resident/Green Card, Birth Certificates or Social Security Cards.

Helpful tips

  • An appointment is required to pick up or drop off applications in person. Call 305-893-6511, Ext. 20003 and schedule an appointment on ​​Monday, Wednesday and Thursday, between the hours of 9:00 AM to 2:00 PM. 
  • Eligible residents who were previously assisted with the first round of ETBRA Program funding can re-apply if the total amount of assistance does not exceed the 6 months or $8,000 maximum allowed.
