Information you’ll need to provide

  • Current executed lease or mortgage statement from the home where you are residing.
  • ID and Social Security card (Applicants only).
  • A letter of justification on why you need this assistance.
  • Documents completed by your landlord including a rent/mortgage form, a W9, and a property management contract (if applicable).

Helpful tips

  • Make an appointment. Offices are open Monday - Friday from 9 am - 5 pm.
  • Submit all of your documents at the same time to avoid misplaces and delays during the processing. The program representatives will need all the documents in order to proceed.
