Information you’ll need to provide

  • I.D. like a driver’s license, passport, or permanent resident card. 
  • Proof of current address such as a utility bill, mortgage statement, or vehicle registration. 
  • Proof of work authorization such as a social security card, U.S. birth certificate, or a U.S. citizen card.  
  • Documentation of all other forms of assistance you may be receiving such as a student. 
  • Income verification for everyone in the household over the age of 18 such as tax returns, pay stubs, or social security benefits. 
  • If applicable, you may be asked to provide a completed Verification of Disability Form
  • Review the Application Document Checklist for a full list of required documentation.
