Information you’ll need to provide

To determine eligibility you will need to provide:

  • Basic contact information including your name, email, phone number and address.
  • Your annual household income from 2020 or 2021.

Once approved for the application you will need to upload:

  • Photo ID
  • Most recent mortgage statement, property tax statement, and utility bill.
  • Proof of payment for insurance

Helpful tips

  • The program will send funds directly to mortgage lenders/services, taxing authorities, insurance companies, etc., not the homeowners. 
  • The maximum amount given per household will be $50,000.
  • Each application is connected to the email address used to apply. If you applying for a client or family member, please note, email addresses can only be connected to one application.
  • If you were denied due to incorrect information email homeownerassistancefund@deo.myflorida.com to have your application updated and reconsidered.
