Requirements

Social Security Number.
Proof that at least 50% of all income comes from working within the music community.
Proof of musician status, for example: a bio, discography, contracts, or guild/union membership card.
Proof of cancellations and bookings from venues.
If you are applying for assistance with medical costs, copies of any outstanding bills that support your hardship claim.


