Information you’ll need to provide

  • Basic personal information including your name, email address, and phone number.
  • Your estimated total household income in 2021.
  • Details about your business including your business name, address, website, and year of incorporation. 
  • A summary of your business and breakdown of number of employees.
  • Your annual gross revenue in 2020/2021 and projected revenue for 2022.
  • Narrative questions such as why are you interested in participating in the Cultivate Small Business program and what type of business do you consider to be your primary competitor.
  • Profit and loss statement from 2021, you can submit screenshots of your Quickbooks or other accounting software.
  • Copy/scan of your ID 
  • Proof of ownership of business (Acceptable forms include Current business license, Articles of Incorporation, or Business registration with your respective city).

Helpful tips

  • Priority will be given to women-, minority- and immigrant-owned businesses in food-related industries. 
  • There is an online info session on Tuesday, April 19 from 12pm-1pm EST. Click here to register.
