Information you’ll need to provide

  • State of FL business registration. 
  • Most recent balance sheet or profit and loss statement. 
  • Current City of Miami Business Tax Receipt, Local Business Tax Receipt or proof of payment. 
  • Full employee list as of January 31, 2020 and date of application. 
  • Copy of most recent utility bill. 
  • Executed lease or mortgage statement. 
  • W-9 form. 
  • Completed ACH form & voided check for payment.

Helpful tips

  • This grant can be used towards the following expenses: commercial lease, commercial mortgage, employee salary or benefits, utility payments, new equipment, or other general expenses critical for business operations. 
  • If this grant is received, you must keep detailed records of the use of the funding to be shared with the Omni CRA.
