Information you’ll need to provide

  • A completed grant application (to be picked up at the City’s Economic Development Office)
  • Copy of Certificate of Status from the Florida Division of Corporations
  • Copy of State of Florida issued picture ID of each business owner with ownership of 20% or more of the business. 
  • Business Tax Returns for 2020 and 2021
  • Copy of City of North Miami Beach issued Certificate of Use/Accessory of Use and Local Business Tax Receipt 
  • Provide Annual Profit & Loss statements for 2020 and 2021 showing proof of loss or reduction of income as a result of COVID-19.
  • Completed & signed W-9 with business name
  • Receipts or bank account statements showing the amount paid for all expenditures not covered by any federal, state, or local COVID-19 relief program that amounts to the maximum amount of assistance which is $5,000.

Helpful tips

  • Note: The application provided on the City’s website is outdated and you will need to pick up a new application in-person to be considered for the grant. 
  • The grant award must be used to cover certain expenses that allowed your businesses to remain viable and operational during the pandemic.
