Information you’ll need to provide

  • Business federal income tax return for the most recent fiscal year. 
  • A way to show your business was or became operational before October 1st, 2019.
  • Bank Statements
  • Business payroll registry 
  • A Duplication of Benefits Affidavit must be completed whether your business has received (or is anticipating) Federal Assistance Funding or not. 
  • Receipts for qualified business expenses you are requesting reimbursement for. 
  • IRS W-9 form 
  • City of Miami Beach Vendor Registration

Helpful tips

  • Be sure to save your work frequently as you fill out your application. You don’t have to complete the application in one sitting. The online application portal allows you to save your work.
  • Don’t worry. If you make a mistake someone you may contact the granting agency and ask for an exception so they allow you to fix it.
