Information you’ll need to provide

  • Florida Picture ID of person applying (business owner or a registered agent). 
  • Business Tax Returns for 2019. 
  • Latest RT-6 form “Florida Department of Revenue Employer’s Quarterly Report”.
  • Proof of loss or reduction of revenue as a result of COVID-19 after March 1, 2020. 
  • City of Miami-issued Certificate of Use/Accessory of Use and Local Business Tax Receipt Card. Must be current and up to date.
  • Proof of your business having a DUNS Number.
  • Proof of current household income for the owner. 
  • Completed and signed W-9 Form matching the business’ name to the one in the application.
  • Proof of payment.

Helpful tips

  • Before you begin the online application process, gather all the required documentation. You will need to upload them to the online application.
  • Click here to find out if your business is located within the City of Miami. After searching your business address, the first two numbers of the Folio will be 01 if you are based within the city's boundaries.
