Information you’ll need to provide

  • Basic contact information (name, phone number, email address, etc.)
  • Information about your business: business name, structure, year founded, website, social media (if applicable), a brief description, etc.
  • Answering the following questions based on which section you would be applying for (you can select more than one).
  • A few narrative questions about your business and why you are interested in the American Dream Rewards.
